Payer & RCM updates

Signals worth bringing into the next revenue-cycle conversation.

A source-linked monitor for payer, policy, coding, enrollment, and revenue-cycle developments—paired with independently written Provenio context, not copied third-party reporting.

This page is for awareness and operational discussion. The original publisher controls its content and should be consulted before any coding, coverage, reimbursement, enrollment, compliance, or regulatory decision.

Reviewed context

What is new—and what to clarify before acting.

Each note below is written by Provenio in its own words after reviewing an approved official source. It is deliberately brief, links to the original, and does not replace the source guidance or a qualified review of your organization’s circumstances.

Coding & documentation

October 1, 2026

FY 2027 ICD-10 code files are in effect.

CMS has made the October code-update files available. A practical next step is to confirm that coding, documentation, charge-capture, and edit workflows are using the applicable files for the date of service. The original CMS files and guidance remain controlling.

Price transparency

October 5, 2026

CMS finalized updates to Transparency in Coverage requirements.

The final rules address payer machine-readable-file standardization, accessibility, and timing. Organizations that work with payer data may want to identify the affected files, ownership, and implementation dates before translating the update into an operational work plan.

Enrollment & plan operations

August 25, 2026

CMS released 2027 Medicare Advantage and Part D enrollment guidance.

CMS says plans are expected to use the updated guidance for requests received on or after January 1, 2027. Teams with enrollment-facing responsibilities can use the source material to identify dependencies, notices, and readiness questions relevant to their own scope.
